This injection is accomplished for selected nerves as they exit the spinal canal. This is used as a diagnostic and/or therapeutic procedure. With the assistance of fluoroscopy, the needle will be placed near the nerve and a local anesthetic agent and/or steroid is injected. It is usually more effective than a simple translaminar epidural steroid as all of the steroid is placed in a very small volume right on the injured nerve. Many patients have an anterior spinal canal problem such as discogenic pain or irritation of the posterior longitudinal ligament. When the steroid is placed via transforaminal or selective nerve root procedure into the front of the canal, it will achieve the best results possible.
